About Us. Homeostasis Therapeutics, LLC, was founded by the parents of a young woman (Caroline) who had suffered from severe anorexia nervosa for 15 years before she was cured by a novel treatment. We are dedicated to the research, treatment, and cure of anorexia nervosa based on the latest scientific understanding that anorexia nervosa may be a curable metabolic condition.

Our Mission. To offer an evidence based, sequential treatment for anorexia nervosa comprised of a high fat metabolic ketogenic diet followed by several ketamine infusions.

“I feel totally freed from the monster that inhabited my mind and body”

-Caroline B.

Our Story.

The Beginning. Caroline’s maternal aunt, Dr. Barbara Scolnick, a Massachusetts physician, with no expertise in eating disorders, became determined to find a better way to treat the disease when she saw the terrible toll anorexia nervosa took on her beloved niece and the ineffectiveness of the current treatment protocol.  The approved treatment then and now is to encourage the patient to eat ALL foods, with the help of family or a support person at home or in a hospital or residential treatment center; receive “talk therapy”, and then resume making one’s own food choices.  The treatment takes months at best and usually years, at great cost, often with many relapses. At worst, it fails and the patient either continues to live a very restricted life or dies from suicide or starvation.

The Science.  Dr. Scolnick studied the science and uncovered hints in the medical literature that nutritional lipids (fats) might unlock the ferocious grip of this horrible illness.  Laboratory rodents have been recognized as a fairly good model for studying human anorexia. Studies showed that some caged rodents given a running wheel and slightly less food than usual, began to run excessively and eventually did not eat. This behavior, known as activity-based anorexia, replicates the self-starvation plus hyperactivity that marks human anorexia nervosa. As long ago as the 1970s, it was observed that activity-based anorexia was interrupted if the animal was fed high fat food, the essence of a metabolic ketogenic diet.  A 2016 study from Dr. Chiye Aoki’s laboratory at NYU demonstrated that rodents reduced self-starvation if they received one infusion of racemic ketamine.  A 1998 human study showed that 9 of 12 patients with chronic anorexia experienced substantial relief from food obsessions and compulsions following IV ketamine infusions. This 20+ year old study was neither repeated nor referenced until it was cited in the Case report that was published describing Caroline’s experience.

The Hope. Dr. Scolnick decided it was possible that a combination of a modified ketogenic diet followed by IV ketamine infusions, something never done before, might be a way out of this disease.  She was unable to interest any eating disorder treatment center or academic institution in undertaking a clinical trial to test a combined ketogenic diet/ketamine treatment method because the knee-jerk response to hearing “keto” was that it would be too likely to cause further weight loss.   At age 30, after having anorexia for half her life, Caroline agreed to be the first person to test this novel method. Closely working together, Dr. Scolnick and Caroline pioneered this treatment.   Remarkably, after 2 months on the diet, and 4 ketamine infusions, Caroline experienced total remission.  She is anorexic free to this day, over three years later, without any further treatment. Weight restored, no anorexic thoughts, no compulsion to exercise incessantly. The Case Report describing the treatment protocol can be found in the July 30, 2020 edition of Frontiers in Psychiatry and is entitled: “Remission from Chronic Anorexia after Ketogenic Diet followed by Ketamine”, https://www.frontiersin.org/articles/10.3389/fpsyt.2020.00763/full

Five Person Clinical Trial.  Was Caroline’s success “a medical miracle”? Was it simply a product of her trust in her aunt and willingness to get better? Homeostasis Therapeutics, LLC was formed to find out.  We funded a “proof of concept” IRB approved five-person clinical trial whose active phase (including 2-3 months of eating a modified ketogenic diet before ketamine treatment and 1 year of monitoring) lasted from April 1, 2021, until June 2022. Dr. Lori Calabrese, the psychiatrist who had administered the ketamine to Caroline, oversaw the trial assisted by: (1) a ketogenic-trained dietician, (2) Dr. Barbara Scolnick, Caroline’s aunt; and (3) Caroline, as the peer counselor, coaching the 5 patients through the treatment.

At six months, four of the five patients, all of whom were chronic anorexia sufferers and had tried other treatment programs, improved and two have remained largely symptom free.  No patient experienced adverse effects related to the treatment. The Case Report describing the study can can be found in Eating and Weight Disorders-Studies on Anorexia, Bulima and Obesity, August 23, 2022. The report is entitled “Ketogenic Diet and Ketamine Infusion Treatment to Target Chronic Persistent Eating Disorder Psychopathology in Anorexia Nervosa: a pilot study” https://rdcu.be/cUdZx

Coming Next.

Larger Clinical Trial.  We are working with The University of California, San Diego to set up a larger, randomized clinical trial using essentially the identical protocol that was used so effectively for Caroline and the clinical trial participants.

Funding. We are interested in obtaining funding to move forward with making the treatment commercially available.
